The Health Records Office only accepts completed health records. Health records are processed in the order in which they are received. Please check the requirements for your program before submitting your complete health record. Students who submit their completed health record after the stated deadline will be subject to additional processing time.
Our office is currently under transition. If you are submitting a complete health record for your new program of study as of 4/7/2023 or after; you will follow the submission instructions listed on the last page of each health record under Forms by Program or click the following link: Immuware Submission Instructions
Students who were admitted into their program of study prior to 4/7/2023, had a complete health record on file with our office for that program prior to 4/7/2023, and are submitting updates, will follow the submission guidelines below.
